Rising Bleeding Cases in United States Infants Linked to Refusal of Vitamin K Shot

Concerns are growing among medical professionals in the United States regarding a rise in cases of internal bleeding among newborns who have not received the standard Vitamin K shot shortly after birth. This alarming trend has been attributed to a refusal of the injection by some parents, leading to potentially deadly consequences.

Several specific cases have been reported, including a seven-week-old infant in Maryland who experienced seizures and another newborn in Kentucky who became lethargic following vomiting. An additional instance involved a baby in Alabama who briefly stopped breathing before passing away. All three had not been administered the recommended Vitamin K injection after their births, as detailed in a recent report.

The situation is underscored by the condition known as Vitamin K Deficiency Bleeding (VKDB), which can pose severe health risks to infants due to insufficient vitamin K in their systems. Vitamin K is essential for blood clotting, and without it, babies are at heightened risk of internal bleeding.

Importance of Vitamin K for Newborns

Vitamin K plays a crucial role in the production of clotting factors that help prevent bleeding in the body. Newborns are born with low levels of this vitamin since minimal amounts are transferred from the mother during pregnancy, and breast milk contains only trace amounts.

As a result, infants are particularly susceptible to dangerous bleeding episodes in the early weeks and months of life. Dr. Rahul Nagpal highlighted that babies lack sufficient clotting factors initially, making the administration of Vitamin K critical for their health. The recommended intramuscular injection is typically given within the first six hours after birth, ideally within the first 24 hours.

Without the Vitamin K shot, infants are significantly more likely to experience VKDB, which can lead to severe complications such as bleeding in the brain, intestines, or other vital organs. Early symptoms may include unexplained bruising, vomiting blood, or lethargy, yet many cases may present without any initial warning signs.

Current Vitamin K Administration Rates in India

In contrast to the US, the issue of parents refusing Vitamin K injections remains relatively rare in India. Nonetheless, some infants still miss this essential shot, primarily due to factors such as home births, lack of awareness, and incorrect information circulating on social media platforms.

Dr. Nagpal stated that while outright refusal of the injection is not a widespread issue in India, instances of hesitancy influenced by online misinformation are on the rise. A 2021 analysis indicated that approximately 62 per cent of newborns received the vitamin K shot, with many still lacking access to adequate healthcare instead of outright refusal.

In the US, misinformation plays a significant role in parental decisions regarding the shot, with some mistakenly viewing it as a vaccine or expressing unfounded fears of links to cancer, despite substantial evidence dismissing such claims. The American Academy of Pediatrics has recommended the vitamin K injection for newborns since 1961.

Risks Associated with Missing the Shot

Medical professionals warn that infants who do not receive the Vitamin K shot are at a dramatically heightened risk of developing severe bleeding disorders. Such infants are estimated to be 81 times more likely to experience VKDB in their first six months of life. The condition manifests in multiple stages, each with varying levels of seriousness.

Early VKDB tends to occur within the first 24 hours and is often associated with medications taken by the mother during pregnancy, while classical VKDB arises between two to seven days post-birth. The most critical form, late VKDB, can transpire from two weeks to six months after delivery, potentially leading to severe brain bleeding.

Dr. Nagpal emphasized the serious implications of late VKDB, noting that a significant percentage of affected infants develop brain bleeding, which can present suddenly with symptoms such as seizures, extreme sleepiness, and collapse. Medical experts stress the importance of evidence-based advice to counter misinformation and promote the life-saving benefits of the Vitamin K shot in newborn care.
