Sonam Wangchuk Remains Conscious Yet Dehydrated at Safdarjung Hospital, Refusing Recommended Fluid Treatments

Sonam Wangchuk, a prominent activist, was admitted to Safdarjung Hospital on Saturday, July 19, due to his ongoing hunger strike. The hospital confirmed that he is conscious and clinically stable but is exhibiting signs of dehydration. According to Medical Superintendent Dr Charu Bamba, despite various advisements from medical staff, Wangchuk continues to refuse both oral and intravenous fluid therapies.

Wangchuk was brought to the hospital at approximately 7:30 am by the Delhi Police as part of a carefully coordinated operation ordered by the Delhi High Court. This admission occurred just two days before a planned march to Parliament, coinciding with the commencement of the Monsoon Session on July 20. Dr Bamba stated that his vital signs, including blood pressure, pulse rate, and oxygen saturation, remain stable, indicating that he is fully conscious and oriented.

Dr Bamba further elaborated on Wangchuk’s condition, noting, “He is definitely showing signs of dehydration. He needs treatment. But despite our counselling, he is refusing treatment.” Wangchuk has declined all forms of rehydration solutions, including electrolytes, both orally and intravenously.

Expert Opinion and Ongoing Monitoring

Dr Akshay Kumar, an Additional Professor in the Emergency Medicine Department at AIIMS, has joined the medical team treating Wangchuk. Dr Bamba confirmed that an expert opinion was sought, which aligned with the existing medical assessments. The treating team is closely observing Wangchuk to encourage him to accept the necessary fluid therapy that could alleviate his dehydration.

The medical team has been in continuous dialogue with Wangchuk, attempting to persuade him to accept the recommended treatments. “We are doing everything possible to ensure he receives the necessary care,” Dr Bamba added, emphasising the need for immediate medical intervention.

In the early evening, the Ministry of Health and Family Welfare released a statement concerning Wangchuk’s health. The statement indicated that, despite relentless efforts from medical staff and independent experts, Wangchuk’s family has not consented to the proposed medical interventions. The ministry confirmed that he remains under strict medical observation.

Medical Risks Associated with Extended Fasting

General physician Dr M. Wali has articulated the potential medical risks linked to prolonged fasting, particularly highlighting the dangers of ketone levels rising in the bloodstream. He explained that while the formation of ketone bodies is a natural response during fasting, excessive ketone levels can lead to severe complications.

“The formation of ketone bodies in the blood is a normal physiological process during fasting,” Dr Wali stated. He pointed out that although individuals might safely undergo fasting for short periods, extending this beyond a threshold can become perilous. He indicated that if ketone levels exceed three mmol/L, serious health risks could arise.

Dr Wali urged that in cases of extreme fasting, medical teams typically recommend nutritional intake to mitigate health risks. However, if individuals decline this recommendation, medical professionals may have to consider forced nutrition to prevent potentially life-threatening conditions.

Ultimately, Dr Wali emphasised the importance of continued monitoring in a hospital setting to ensure all vital parameters, such as blood pressure, hydration status, and oxygen levels, are maintained within safe limits. He underscored the necessity of appropriate medical intervention where required, especially in individuals involved in extended hunger strikes.
