Parkinson’s Disease May Begin Years Before Symptoms Appear

Parkinson’s disease is becoming a major neurological issue in India, with current estimates suggesting that approximately 7.5 lakh individuals are living with the condition. Over the past two decades, there has been a reported 21 per cent increase in the prevalence of Parkinson’s, highlighting its growing significance as a public health concern. Alarmingly, studies indicate that the onset of the disease in Indian patients can occur nearly ten years earlier than the global average, emphasising the urgent need for awareness and early detection.

Understanding the Prodromal Stage

The initial phase of Parkinson’s disease, referred to as the prodromal stage, often goes unnoticed. During this time, non-motor symptoms may emerge long before the characteristic signs such as tremors or muscle stiffness present themselves. Research indicates these early markers could develop between 15 to 20 years prior to an official diagnosis, complicating efforts to identify the disease early.

Commonly disregarded, these initial symptoms can include persistent constipation, a diminished or absent sense of smell, as well as feelings of depression and anxiety. Sleep disturbances and general gastrointestinal discomfort are also prevalent. Since these issues are fairly common in the general population, they typically do not correlate strongly with Parkinson’s in its early stages, leading to difficulties in early recognition and intervention.

Recent research has provided insight into these overlooked early symptoms. For example, a study conducted in 2025 in Egypt revealed that 22 per cent of subjects with Parkinson’s experienced constipation prior to the onset of motor symptoms. Concurrently, a study in Odisha reported that all participants had gastrointestinal problems, and substantial proportions also reported symptoms of depression, anxiety, sleep difficulties, and loss of smell. Many noted these issues years before receiving an official diagnosis, indicating a pressing need for greater awareness regarding the prodromal phase.

The Mechanisms Behind Early Symptoms

Scientific investigations suggest that the early signs of Parkinson’s disease may be connected to the gradual accumulation of a protein known as alpha-synuclein in the brain. This accumulation primarily initially affects areas that regulate sleep, sense of smell, and gut function. It is only after these regions are disrupted that movement-related areas become impacted, resulting in the observable symptoms of tremors, rigidity, and slowed movement. At that point, clinicians are able to provide a clinical diagnosis of Parkinson’s.

Despite growing awareness, early diagnosis of Parkinson’s remains a formidable challenge. Medical professionals in India often approach patients cautiously, particularly when considering non-motor symptoms alone as evidence for diagnosis. This hesitance stems from the potential psychological distress that can arise from an incorrect or premature diagnosis. The non-specific nature of early signs renders them ambiguous and complicates the diagnostic process.

Furthermore, the situation in India is exacerbated by the tendency for Parkinson’s to develop earlier than in other regions globally. With an increasing prevalence and an earlier onset age, medical practitioners face the complex task of identifying early warning signs while also judiciously avoiding overdiagnosis, which can lead to unnecessary concern for patients.

The Path Forward

Fostering awareness that Parkinson’s disease often begins without noticeable tremors is crucial. Greater attention to persistent and unexplained non-motor symptoms may facilitate earlier medical evaluations, although establishing a definitive diagnosis could still require time. As the understanding of this condition evolves, a more nuanced approach to detection and management is necessary to support those possibly affected.
