Is Oral Sex Replacing Smoking as the Leading Cause of Throat Cancer?

Historically, throat cancer in India has been closely linked to tobacco use, particularly smoking and chewing tobacco. However, doctors are now observing an increase in cases associated with the human papillomavirus (HPV), a virus frequently transmitted through intimate interactions, including oral sex.

This change is prompting renewed discussions on sexual health, HPV awareness, and the importance of vaccination. While smoking continues to be the foremost cause of throat cancer in India, countries like the United States and the United Kingdom have reported HPV infections from oral sex surpassing tobacco use as the leading factor.

The Growing Concern of HPV

HPV is recognized as one of the most prevalent viral infections globally. Although many HPV infections resolve on their own, certain high-risk strains, notably HPV-16 and HPV-18, can persist and lead to various cancers. These strains are primarily known for their connection to cervical cancer but have also been linked to oropharyngeal cancers that impact the tonsils and the base of the tongue. Medical practitioners assert that HPV can be transmitted through oral sex, allowing the virus to infect throat tissues. In several Western nations, HPV has already replaced smoking as the principal cause of oropharyngeal cancers.

Current Data on Throat Cancer in India

India faces a significant burden of head and neck cancers, which represent approximately one-third of all cancer cases in the country, marking one of the highest rates globally. However, health professionals maintain that tobacco and smokeless tobacco are still the leading culprits for throat cancer in India.

Although there is an uptick in HPV-related cancers, these still constitute a smaller percentage compared to those linked to tobacco. Doctors are increasingly diagnosing throat cancer in men aged 40 to 50, many of whom lack a history of smoking or tobacco use, raising concerns regarding the role of HPV in these cases.

Identifying Early Symptoms

Throat cancer arises when genetic mutations cause abnormal cell growth in the throat. Initial symptoms may be subtle but can include persistent hoarseness, a sore throat lasting more than two weeks, difficulties with swallowing, ear pain, lumps in the neck or throat, and unexplained weight loss. Medical experts stress the importance of early detection, as it significantly enhances treatment outcomes and underscores the necessity of awareness alongside preventive measures.

Dr. Subhramania Iyer, the Head of Head and Neck Cancer at Amrita Hospital in Faridabad, has emphasized that 90% of head and neck cancers are related to lifestyle choices. He stated, “Cancer is curable if detected early. The importance of early diagnosis and timely treatment cannot be overemphasized. It is perhaps the single most important factor that determines outcomes.”

Focus on HPV Vaccination

The increasing discussion surrounding HPV infections coincides with the Indian government’s efforts to expand HPV vaccination programs. On February 28, the Union Health Ministry launched a campaign aimed at vaccinating 14-year-old girls to reduce cervical cancer rates in the country. This vaccination effort could also indirectly mitigate the incidence of other HPV-related cancers, including throat cancer. Many health specialists are now advocating for the vaccination of boys as well, since HPV is transmitted through sexual contact and can impact individuals of all genders.
