Merck and Moderna Personalised Cancer Vaccine Achieves Major Trial Milestone

Recent developments in cancer treatment have reached a significant milestone with the announcement from Merck and Moderna regarding their personalised mRNA cancer vaccine, referred to as intismeran autogene. This vaccine has successfully completed Phase 3 trials for melanoma, marking a historic achievement as it is the first individualised cancer vaccine to reach this stage of testing. The announcement occurred on August 19, revealing that the vaccine, when combined with Keytruda, a well-established immunotherapy drug from Merck, has shown promising results.

Mechanism of the Vaccine

The term “personalised” is exemplified in this treatment approach. Following the surgical removal of a patient’s tumour, medical professionals analyse a sample to sequence its DNA and identify unique mutations characteristic of that individual’s cancer. This specific methodology ensures that no two patients receive identical formulations. Up to 34 mutations specific to each patient can be encoded into a single mRNA injection, which is tailored exclusively for that person. Once administered, the vaccine prompts the immune system to identify and target any remaining cancer cells harbouring those particular mutations.

Traditionally, Keytruda has been the primary treatment for high-risk melanoma patients post-surgery. While it effectively enhances the immune response, a significant number of patients experience a relapse, often within the initial two years. This ongoing challenge is primarily due to the cancer spreading to more distant regions of the body rather than remaining local, highlighting the need for an advanced therapeutic option.

The Phase 3 trial, labelled INTerpath-001, included 1,137 participants with high-risk melanoma, specifically those with stages IIB through IV of the disease. Participants were randomly assigned to receive either the personalised vaccine in combination with Keytruda, or Keytruda alone over a treatment period of approximately one year. The study’s primary endpoints measured recurrence-free survival—the duration before cancer returned—and distant metastasis-free survival—the time before the cancer spread to distant sites in the body. Initial findings suggest that the combination therapy substantially improved outcomes compared to the use of Keytruda alone.

Broader Implications Beyond Melanoma

The relevance of this trial extends beyond melanoma. In 2022, over 330,000 new melanoma cases were reported globally, with the United States projected to see more than 112,000 new diagnoses and approximately 8,500 deaths this year alone. However, the breakthrough lies in the technology underpinning the personalised vaccine, which has potential applications beyond melanoma. Merck and Moderna are reportedly initiating trials for the same personalised approach for other cancers, including lung, bladder, and kidney cancer, reflecting the broader applicability of this innovative treatment.

The CEO of Moderna, Stphane Bancel, has drawn parallels between the significance of this development and the announcement of their Covid-19 vaccine trial results, emphasising a pivotal moment in cancer care. The implications of personalised medicine through the vaccine platform could potentially reshape cancer treatment methodologies across multiple cancer types.

It is noteworthy that the data shared to date is preliminary and has not undergone peer review or publication in a medical journal. Detailed efficacy statistics will be disclosed at an upcoming international medical conference, and the trial will continue to monitor overall survival rates, a key benchmark in cancer research. If substantiated, this approach could demonstrate that cancer therapies can indeed be customised for individual patients based on their unique tumour characteristics.
